Student supervision begins at 7:45 a.m. Children who will be eating
breakfast
may arrive at 7:15. These students will be held in the cafeteria until
7:45.
Students will be picked
up at 2:30pm following dismissal from school.
Under no
circumstances is a student to leave the school prior to dismissal time
without
permission from the office. Students are not to be on campus after
school hours
unless it is for a supervised after school program.
RELEASE OF PUPILS
In the interest of
security, any students who must leave before dismissal time
due to doctor's appointment, illness, etc. must be released through the
school
office. Parent or guardian will be requested to sign the Student
Sign-Out book.
Your child will be called to the office to check out.
If someone other than a parent or guardian wishes to check out a child,
a note
from the parent or guardian will be necessary before we can release your
child
to them. Only authorized persons will be allowed to remove your child
from
school. Please keep your child's emergency card up-to-date to prevent
delays.
ILLNESS OF ACCIDENT AT
SCHOOL
In case of illness or
accident at school, parents will be notified immediately. No
child will be allowed to remain at school with fever. As our school is not
equipped or staffed to handle ill children for long periods of time, we
request
that you pick up your child as soon as possible when contacted. The
following
information should be kept on file in the school office and be UPDATED
as
necessary:
· Name, address, home phone and business phone of parent or guardian
· Emergency numbers should also be on file in case parents cannot be
reached
(Relative or Friend)
ABSENCES AND TARDINESS
Florida State Law
requires that a student be in attendance every school day
except for personal illness. It is required by law that parents explain
the cause of
a child's absence or tardiness. If your child is absent for any reason,
you must
call the school attendance phone line at 239-1321. We will call you if
your child
is absent and you have not called the attendance line.
Ten or more days of unexcused absence is considered excessive and would
warrant a referral made to the school social worker for their
assistance.
Students who are absent in excess of 20 days may be required to attend
the
summer extended year.
All students who are late must report to the office for a tardy pass
before going
to class. Students who are tardy should bring in a note stating why they
are late.
We discourage excessive tardiness. Children miss important instruction
if they
arrive late. Being on time sets the tone for a good day.
ADMINISTRATION OF
MEDICINE
Authorized school
personnel will assist students in the administration of
prescription when the following conditions have been met:
Only prescription medication will be administered at school
Over the counter or sample medication must be accompanied by orders from
the physician. Without these orders, the medication will not be
administered.
Parents must deliver the medication to the school office in the
container it was
purchased. At no time should a child transport medication of any kind.
Parents Authorization Forms must be completed in order for the student
to take
medication.
BREAKFAST AND LUNCH
PROGRAM
Our school starts with
breakfast being served at 7:15 a.m. each morning.
Students are encouraged to eat a nourishing breakfast to assure
classroom
success.
A well-balanced lunch including milk and juice will be available daily to all
students
who want to purchase a school lunch. Each student is responsible for
his/her
own lunch money, payable daily weekly, or monthly. Make checks payable
to
Re-Birth Academy. Please put your child's name and teacher's
name
on lower left portion of the check. Lunches should be paid at the
lunchroom
before school begins.
In an emergency, a child may charge a lunch for one day, and pay the
next
school day.
APPLICATIONS FOR FREE
AND REDUCED MEALS ARE
AVAILABLE THROUGH THE OFFICE.
PHYSICAL EDUCATION
All students are
expected to participate in physical activities. Students are not
excused from such classes without a written statement from a doctor.
